Six highest-denial categories: crowns, partial dentures, complete dentures, additional scaling units, periodontal services, major restorative. Categories, not procedure codes, because codes and preauth flags differ by province and provider type.

Yes/no, dates, and counts — never a chart note. Conditional questions appear only when they apply, and an excluded service ends the check on the spot with the Appendix E citation.
